2017 Sports Online Registration Form
Below is what your child will need prior to the start of the season.

  1. Football – Early Bird Registration $135 [Must be paid in full by June 30th];
    Regular Registration $175
  2. Cheerleader - Early Bird $100 [Must be paid in full by June 30th]; Regular Registration $125 [All fees are Non-refundable after August 10th].
  3. Basketball - Registration $100 [No Refunds after December 1st]
  4. $100 Football /$50 Cheer Equipment Security Deposit [Money Order Only!!! Deposit returned at equipment collection.]
  5. Maryland MVA Non-Drivers ID, or Official Gov’t I.D. Required, and (DC Residents) Copy of Original Birth Certificate.
  6. See Mandatory Volunteer (Parents) and Fundraising – see detail below
  7. Signed: Registration form, Parent and Player Code of Conduct Form

IMPORTANT POLICY INFORMATION:

By Signing I/WE AGREE TO THE FOLLOWING TERMS:

As the parent (or legal guardian) of the above named minor, I have verified that the information on this form is correct and I grant permission for this minor to participate in all of the activities of this sports program. I assume all risks and hazards incidental to such participation, including risk of serious injury including transportation to and from all activities. This care may be given under whatever conditions are necessary to preserve the life, limb, or well being of my dependent. To the best of my knowledge, there are no physical or other conditions, which will interfere with my child’s participation and do hereby release and waive all claims against Takoma Park Boys and Girls Club, Inc,. it’s officers, directors, coaches, sponsors , volunteers and other participants. I further grant permission for emergency first aid to be given to my child in case of injury. Also, I give my permission for photographs, slides, video or audiotapes to be taken of my child to be used for our website, and/or public relation purposes promoting TPBGC.

**Mandatory: ** Volunteer Parents – (5 hours) Field Preparation, Concession Stand, Team Parent, Assist calling phone tree, or donation (food, drink, snack and ice for the game.) **Fundraising- must participate in Two (2) fundraising activities or give TPBGC a $100 check, money order or cash as a fundraising donation.

To prevent dehydration, we require each child bring one gallon of water every Tuesday to practice.


I agree to pay the fee for registration and to return the uniform issued to my child in a clean and same condition as received. Refunds will be given minus a $25 service charge, up until 1 week prior to the first game of the sport activity season. If in the event I do not return all equipment at the end of sports activity season then I, authorize the forfeiture of my $100.00 uniform security deposit to replace the uniform and additional cost associated with the replacement. Two (2) attempts will be made by the Takoma Park Boys & Girls Club, Inc., to obtain the equipment.

A $25 Late Fee for applications received after the first game of sport activity season.

I also attest that the above information is true and correct to the best of my knowledge and have been informed that knowingly falsifying registration content may result in sanctions against said child's participation.
